Renew, vary or update a Dangerous Goods vehicle licence

This form can be used to apply for a replacement NSW Dangerous Goods Driver Licence issued under the Dangerous Goods (Road and Rail Transport) Act 2008 that was lost, stolen, damaged or destroyed. This form can also be used to request a replacement licence if you did not receive your licence or if it has a printing error.
